Established in the fall of 2011, Memorial’s Patient/Family Advisory Council (PFAC) was designed to enhance patient and family collaboration with hospital administration, management, physicians and staff in creating a patient/family-centered culture to fulfill Memorial’s mission of providing exceptional healthcare and compassionate service.
"The Advisory Council was established to promote respectful, effective partnerships between patients, families, healthcare providers and administration,” said Cheryl Creasy, vice president of risk management at Memorial. “We feel this is a vital link between Memorial and the community.
“Our goal is to enhance the overall patient/family experience at Memorial through evaluation, consultation and recommendation.”
One of the first projects Memorial’s PFAC was charged with was evaluating the discharge process and materials. The Council, comprised of men and women from throughout Memorial’s service area, learned about the steps necessary to discharge a patient from the hospital as well as reviewed Memorial’s Written Discharge Instruction Booklet.
“The Council members offered a number of good suggestions about various aspects of the process that we will be able to implement,” Creasy added.
Other projects the PFAC could be asked to weigh in on include such things as wayfinding, patient room design for Memorial Hospital-East and food service as well as other process improvement initiatives that relate to delivering patient/family-centered care.
