Referral required
Patients must be referred to Memorial Home Care by their physician, case manager, hospital discharge planner, and/or social worker. The care provided is prescribed to address illness, chronic condition, or disability. Home care services are provided on a part-time basis and are covered by Medicare, Medicaid, and most managed care and indemnity insurance plans.
