Memorial Senior Care offers outpatient group and individual counseling for senior adults. Seniors often face unique problems, which can trigger depression or anxiety. At Memorial Senior Care, we take a team approach, utilizing a psychiatrist, nurse, and licensed therapist to develop individualized treatment plans based on client needs. Reports are provided to other involved medical professionals on a routine basis.

Admission and referral
To make a referral to Memorial Senior Care, call our office at (618) 257-5900. Our staff will request some initial information and promptly schedule an evaluation for the person being referred. All initial screenings are complimentary. We work closely with referring healthcare professionals so that timely communications can take place with families, physicians, social service agencies, and others.
Transportation is available. No referral is necessary; the patient or a family member may call for an appointment. Medicare and insurance accepted.
Memory Clinic
The Memory Clinic, available through Memorial Senior Care, focuses on returning individuals to the peak of their capacity through a comprehensive assessment and treatment plan that includes:
- Cognitive assessments designed by SIU School of Medicine
- Diagnostic imaging and lab tests offered as indicated
- All results reviewed by Dr. Mark Freeman, Memorial Senior Care Medical Director
- Family sessions to educate and offer treatment options
- Medication management and follow-up care as indicated
- Diagnostic summary sent to primary care physician
Memorial’s Memory Clinic is certified by SIU School of Medicine and the Illinois Department of Public Health, and is the only one of its kind in the Metro East area.
